We discuss your concerns, symptoms and questions so you can approach the appointment calm and prepared to talk to your care team. I provide you with a clear written summary of our conversation which you can take to your appointment.

In Person Appointment Advocacy

In addition to your appointment preparation, I attend your appointment to ensure your health care needs are advocated for. I provide a summary of the appointment, including next steps recommended, for you and your authorized care partners.

Full Care Coordination

A calm, organized system of care personalized for your medical needs. I track your appointments, referrals and follow ups making sure nothing falls between the cracks. I disseminate complex health information ensuring you and your designated care partners can make confident health care decisions. Everything is tracked in a centralized care binder (digital and physical) and as needed wellness check ins are included to ensure everyone feels informed and confident in their care plan.

No, this is a private pay service which enables us to keep the focus on you and your personal needs.
